Introduction to T2D: Pre-course questionnaire

As you start the course, we want you to think about what your goals are and how you can go about achieving them. They might include learning more about type 2 diabetes and how it might affect you, or learning how to manage type 2 diabetes by setting diet and exercise targets.
We hope this course can help you in achieving your goals. We hope to support you in improving your knowledge regarding type 2 diabetes and give you the confidence to do the things you want while looking out for your health.
At a few points in this course, we will prompt you to set goals or think about the goals you have already set. At the end of the course, we will check in again to see if your learning goals have been achieved.
This course is designed for people who have type 2 diabetes or care for people with type 2 diabetes. If none of the criteria below applies to you then we do not recommend you continue with My Type 2 Diabetes. Feel free to visit the homepage to see what other courses we offer.
